Diaper rash at 14 months is more often caused not by the brand but by skin irritation from urine, stool, moisture, or wipes, especially if you’ve already changed diapers and it didn’t get better. For now it’s better to change the diaper more often, wash with water without soap each time, dry the skin well and apply a thick barrier cream with zinc or a thick layer of petroleum jelly, plus give the skin some “air time” without a diaper. If the rash is bright red, weeping, has small pimples around it or the redness goes into the folds, it may be a fungal irritation, and then a regular cream often won’t be enough — see a pediatrician to get the right treatment. Seek urgent care if fever, blisters, pustules, severe pain appear, or the rash spreads quickly.

Supermoms, We went to the pediatrician; the pediatrician says they can't say what it is. They told us to see a dermatologist — appointments at the free clinic are booked two weeks ahead, and at the private clinic the doctor's next available appointment is only on 19.10.26.

Хадиджа, The appointment on 19.10 for a rash only under the diaper can usually wait if Milana is alert, has no fever, the rash isn’t painful and isn’t spreading quickly. Before seeing the dermatologist, stop using wet wipes and soap, clean with water, dry thoroughly, and apply a thick zinc barrier cream at every diaper change without rubbing it in. If blisters, pustules, oozing with an unpleasant smell, severe pain, fever, or the rash spreading beyond the diaper area appear, it’s better not to wait for the appointment and to see the on-call pediatrician or go to urgent care.

Хадиджа, Akriderm under a diaper without an exact diagnosis can actually not help and sometimes make things worse: it contains a steroid component, and under a diaper the skin is trapped and macerated, so if there’s a fungal irritation it can spread. I wouldn’t keep applying it blindly and would contact the pediatrician again at least, saying there was no reaction to the steroid cream. Until the exam, use only water, drying, air baths, and a thick protective zinc cream. If the redness is bright, with dots or little pimples around the edges and extends into the folds, you need a dermatologist sooner — try to find any nearby appointment window, because that’s often what candidal diaper rash looks like.

Хадиджа, Right — if there's no improvement on Akriderm, it's better not to wait and have the skin examined in person, especially in the diaper area. Go to urgent care/triage immediately, don't wait for an answer, if Milana is irritable from pain, has a fever, oozing, pustules, blisters, or the rash is spreading quickly. For now it's better not to apply anything new except a barrier cream so the doctor can more easily assess the situation.

Хадиджа, The fact that Milana is calm, running and playing is a very good sign: with a serious inflammation children usually protect that area, cry when urinating or during diaper changes, and eat less. Redness under the diaper can last several days, especially if the skin is already irritated or a yeast infection has developed, so an in-person examination is still the right decision. Before seeing the doctor it's better to stick to simple care and not switch many products at once, to avoid adding a contact reaction. You're doing everything sensibly; now it's just necessary to determine whether this is ordinary diaper rash or if a different treatment is already required.

Хадиджа, From the description — that Milana is active and has no fever — this really sounds more like a local skin irritation rather than something dangerous. At the appointment, be sure to say you've already been using Akriderm and it didn't help — that will help the doctor more quickly distinguish diaper rash from a fungal or contact dermatitis. Until the exam, it's better not to add any new ointments.
